Athlete Factory Participant Waiver, Release and Assumption of Risk (v1)

This waiver is a placeholder drafted for a Canadian (Ontario) youth athletic program. It has not been reviewed by a lawyer and must be reviewed and approved by the client before it governs real registrations.

1. Assumption of Risk

I understand that athletic training involves inherent risks of injury, including but not limited to sprains, strains, fractures, and, in rare cases, more serious injury. I voluntarily assume all such risks on behalf of the athlete named in this registration.

2. Release of Liability

To the fullest extent permitted by the laws of Ontario, I release Athlete Factory, its coaches, staff and facility partners from liability for injury arising from participation, except where caused by gross negligence or wilful misconduct.

3. Medical Treatment

I authorize Athlete Factory staff to arrange necessary emergency medical treatment for the athlete if I cannot be reached, and I am responsible for any resulting costs.

4. Photo and Video (see separate consent)

This waiver does not itself grant photo/video permission — that consent is collected separately during registration, with a private (coaching) option and a distinct, optional public (marketing) option.

5. Acknowledgement

By signing below I confirm I am the athlete's parent or legal guardian, that I have read this waiver in full, and that I agree to its terms.
